being conducted by the
Dana-Farber Cancer Institute in Boston, MA  The survey will be available for the next three month from

Thank you in advance for completing this questionnaire.  We hope to learn how breast cancer
has affected men's lives.  We will be asking you questions about the following:

 your background and medical history
 anxiety and feelings about your breast cancer diagnosis and treatment
 sources of support for you
 symptoms related to your diagnosis and treatment
 your willingness to participate in future research

All of your answers will be kept private.  Please try to answer every question, but it is OK if you skip any questions that you do not feel
comfortable answering.  We hope that this information will help us to learn more about the impact of breast cancer on men's lives and
to develop services to assist men diagnosed with breast cancer.

Please contact Dr. Kathryn Ruddy at 617-632-4587 with any questions.  If you have questions about your rights as a research
subject, please contact the Office for Human Research Studies at the Dana-Farber/Harvard Cancer Center at (617) 632-3029.
