Out of the Shadow of Pink along with A Man's Pink have been invited to participate in a male breast cancer research program which is
being conducted by the Dana-Farber Cancer Institute in Boston, MA The survey will be available for the next three month from
November 10, 2011 through February 10, 2011.
The first stage of this program involves a survey to be completed by male breast cancer patients and survivors which focuses on the
physical and psychological consequences of male breast cancer diagnoses.

Thank you in advance for completing this questionnaire. We hope to learn how breast cancer
has affected men's lives. We will be asking you questions about the following:
your background and medical history
anxiety and feelings about your breast cancer diagnosis and treatment
sources of support for you
symptoms related to your diagnosis and treatment
your willingness to participate in future research
All of your answers will be kept private. Please try to answer every question, but it is OK if you skip any questions that you do not feel
comfortable answering. We hope that this information will help us to learn more about the impact of breast cancer on men's lives and
